Appointments
You may call in for appointments between the hours of 9:00am and 5:00pm. Please let us know if you have a preference as to day or time.
Emergencies After Hours
We are on call 24 hours a day for prompt response to any emergency. For emergencies occurring outside of our normal office hours, simply call our office number 770-214-2121. By doing this you will reach our answering service. Tell the person your name, phone number and problem so they will immediately relay the message to us.
Medical Records and Confidentiality
All your medical records are confidential. We WILL NOT release your records or any part of them without a written authorization received from the patient.
Parental Consent
Minor patients must be accompanied by a parent or legal guardian. A consent to treat form must be signed by the parent or legal guardian before treatment is rendered.
Prescriptions
We ask that you call during office hours, preferably in the morning, to allow time to review the chart and phone the pharmacist. You will need to provide the name and phone number of the pharmacy you use and if you are allergic to any medicines.
Returning Phone Messages
We will return your calls for routine lab and other concerns and to answer questions throughout the day. Please be sure to give us a phone number where you can be reached.
Cell Phone Usage
Cell phones are permitted in the front waiting area only. All cell phones must be turned off upon entering the treatment area. We also ask that you refrain from using your cell phone at the check-in and check-out areas so that you may have the best possible communication with our providers and staff.
Miscellaneous
There is a $5.00 fee for the completion of disability or FMLA forms. There is a $25.00 returned check fee. Photo ID required.
