Before your first appointment, you need to follow the link below to fill out your patient history and convey any problems you are having or wish to discuss. This information is recorded in your electronic medical record and is available for your provider at the time of your appointment.
You should have been provided by phone or letter with a login ID and password. Call the office if you do not have a login ID or password.
